Client Forms
All client forms are sent via PocketSuite. PocketSuite is the app used to schedule appointments and make payments. It’s also HIPAA-compliant, so your personal information stays safe. Once you’re finished completing them, feel free to take a picture of the completed forms and return them if you don’t have a means to manipulate PDFs.
What To Expect
During your initial session at 321 Deep Sleep, your hypnotherapist will ask questions to build a foundation. The first session is between 90 minutes - 2 hours. All sessions are done virtually via Zoom. Follow up sessions are 45 minutes - 1 hour. Pricing is discussed during the free 15 - 20 minute consult call.

During the consult, the presenting issue, what you are looking to get out of your session, package pricing, and whether hypnotherapy is a good fit will be discussed. If the decision is to move forward, please click the link to schedule an appointment. Once the appointment has been scheduled, your hypnotherapist will send new client paperwork to be completed prior to the first session via PocketSuite.
